To provide an outreach and support service specifically for elderly African Caribbean people living in Central and South Manchester and Trafford. In collaboration with service providers and community-based networks promote skills in clients and their carers in order to maximise independent living and maintenance in their local community.

Principal Outreach Tasks

post hospital follow up visits to elderly African Caribbean people at home to provide support and information about services available within the area

Assisting with interpretation/advocacy on behalf of elderly African Caribbean people.

Making a contribution with other professionals to enable the provision of culturally appropriate services to elderly African Caribbean clients.

Creating a link between service providers and community based networks in order to enable elderly African Caribbean people to sustain independent living at home.

Undertaking supportive hospital visits to sick elderly African Caribbean people to provide input into the Care Program assessment from a culturally appropriate perspective.

Facilitating elderly African Caribbean users and carers to take up the service provision of the ACCG.

Undertaking home visits to provide support and information on care packages and service provision available to elderly African Caribbean people.

Facilitating the services of the ACCG to become a part of culturally appropriate mainstream

Provision for elderly African Caribbean people.

Establishing a system of continuous support through exchange of information and education with both voluntary and statutory agencies.

Working with Health and Social Services elderly care providers to become more sensitive to the needs of African Caribbean elderly in the area of benefit.

Maintaining detailed and up to date client reports and records and database information.

Preparing reports (as directed by the line manager) to provide accountability of the Community outreach Service to its funders, sponsors and the ACCG Board of Directors.

Attending meetings as arranged with Health and Social Services Care Teams to update on issues relevant to the ACCG’s Outreach Service.

Acting as key worker at the point of closure of the case by social workers.

Participating in social, educational and cultural activities, in a flexible manner, which promote the African Caribbean Care Group.

Participating in staff and community meetings and support systems within the organisation.

Participating in staff development, including induction, on the job training and attendance at relevant training sessions.

Setting priorities and working from them while remaining responsive to the needs of clients and the organisation.

Acting for the Executive Director in his/her absence from the Organisation.
